To apply to K–12 and LSS programs, please complete the application form linked below. Please note, the primary applicant is the student’s parent or guardian. For adult students (those who are 19 years of age as of July 1), the primary applicant may be the student.
For each new applicant, please include the following documents:
For the Student:
- Birth Certificate or Canadian Passport (for applicants who are not Canadian citizens, valid permanent residence card or student VISA are acceptable)
- Latest Report Card
- Latest Individual Education Plan (if applicable)
- Applications to Learning Support Services (special education) require a copy of psycho-educational assessment or medical assessment stating the student’s designation category
- Documentation pertaining to the custody of the student (if applicable)
For the Parent:
- Proof of Residence in British Columbia (driver’s license, utility bill, credit card invoice, mortgage statement, municipal tax bill, notary authorized letter, proof of purchase, or rental agreement)
You are able to submit your application without uploading documents however, new applications will not be processed without the required supporting documents.
You may provide documents by any of the following options:

- Fax to 604-575-8565
- Hand-deliver to the TLA office (103 – 17688 66 Ave, Surrey, BC V3S 7X1)
Documents will not be accepted if illegible.
